A signed waiver by event participant or parent/guardian will be required before you can download your QR code. A link to obtain waivers for all registrants will be shown on the confirmation page.

ASSUMPTION OF RISK

I acknowledge that participation in basketball activities involves inherent risks, including but not limited to physical injury, illness, or other unforeseen incidents. I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a result of participation in this event.

RELEASE OF LIABILITY

I, on behalf of myself, my team, players, parents/guardians, and any representatives, hereby release and hold harmless Icon Hoops, its directors, staff, volunteers, sponsors, venue operators, officials, and affiliates from any and all claims, demands, actions, or causes of action arising out of or related to participation in this event.

MEDICAL AUTHORIZATION

I authorize Icon Hoops staff or designated medical personnel to obtain emergency medical treatment for any player on my team if deemed necessary. I understand that Icon Hoops is not responsible for medical expenses incurred.

MEDIA RELEASE

I grant Icon Hoops permission to photograph, video record, and otherwise capture images or footage of players and team members during the event. I agree that these materials may be used for promotional, marketing, social media, website, and media purposes without compensation.

GRADE & ELIGIBILITY VERIFICATION

I certify that all players on my team meet the grade requirements for this event. Any grade exceptions must be properly documented and approved prior to competition. Failure to provide accurate information may result in disqualification without refund.

CODE OF CONDUCT

I understand that Icon Hoops reserves the right to remove any player, coach, or spectator whose behavior is deemed unsportsmanlike, unsafe, or disruptive. Removal may occur without refund.
